Thanks for a well researched and informative post Marsha, though this question seems for the unit seems very familiar 🙂 Thanks for discussing meaningful use and incentives that come with EHR implementation. How does adoption affect efficiency, privacy, data storage, ease of record access, and cost? This is from Unit 5 initial DB.

An E.H.R. means different things to different organizations and the people within them.  Smaller physician practices think of it as a key system for documentation and coding support.  For some, it is merely a document imaging system that provides access to paper-based records.  For lager hospitals and clinics, it consists of a massive and costly undertaking or merging data from source systems redesigning processes, building decision support and gaining options (Amatayakul, 2004). 
The Institute of Medicine offered a concise definition on the computer based patient record and had a vision for the E.H.R. system that consists of collecting data from multiple sources and it is used by clinicians as the primary source of information at the point of patient care in order to provide a decision support for evidence-based health care (Amatayakul, 2004).
The achievement of this vision has had its challenges since no standards define what an E.H.R should do and this left the system open for many interpretations.  However, the Institute of Medicine managed to identify eight core functions for the EHRs that have been woven into the EHR-System Functional Model Draft Standard that include health information and data, results management, order entry management, decision support, electronic communication and connectivity, patient support, administrative processes and reporting and population health management (Amatayakul, 2004)
